Setting up an Appointment

What to bring to your first appointment

• Valid Identification
• $10 co-pay

To help us determine your eligibility for Sliding Scale Fees, we need documentation of your Household Income.
You will need to provide two of the following documents:

• Recent Tax Return
• Recording Royalty Statement
• 1099 Form
• W-2 form
• Most recent pay check stub
• Notarized letter from your bandleader, club owner or manager/agent or employer.

If you do have health insurance, you will need that information as well. More than 90% of our musician patients don’t have health insurance.

NOMC Patient Eligibility

We offer occupational and comprehensive health services to all New Orleans musicians, vocalists, music tradition bearers, Mardi Gras Indians and their household members over the age of 17. We accept medical insurance, though those without it will be treated regardless of their ability to pay. For those with insurance, but a high deductible we offer our full range of medical services on a sliding scale fee and low-cost, prescriptions.
